We require a completed referral form and a parental/carer consent form for each person before we can start our mentoring.
A copy of any extra documents are also useful – for example, EHCP, risk assessments or CHIN plans that you feel would benefit us in mentoring them more effectively if they have these.
The more information shared following GDPR guidelines, the better to help us to match the right mentor for the young person to work towards a successful engagement.
